How to Prepare for Your First Visit
To help us make the most of your time, here are a few simple things to bring or prepare before your appointment:
- A valid photo ID
- Your dental insurance card (if applicable)

- A complete medical and dental history (or fill out new patient forms ahead of time)

- Records or X-rays from a previous dentist (if available)
Don't worry if you don't have everything. Our team is happy to help you gather what's needed and make the process as smooth as possible.

To save you time at your first visit, we recommend filling out our new patient forms ahead of your appointment. You can download and complete them at home, then bring them with you, or fill them out digitally before you arrive.
If you prefer, you're welcome to arrive 15 minutes early on the day of your visit and complete the forms in our office. Either way, our team is here to help if you have any questions.
